Vendor Application

The Knoxville Utilities Board (KUB) requires a vendor to complete this Vendor Application Form and attach their W-9 to receive purchase orders and payments from KUB. In addition, vendors can provide updates to their contact information, payment address, and business classification status. 

KUB is a Tax-Exempt Government entity. Please code all invoices to exclude sales tax. If you need a current copy of our Tax Exemption Certificate and/or W-9, please contact us at accountspayable@kub.org.
